A young woman with medical background of congenital aortic valve stenosis presents to the emergency room with heart failure symptoms. The cardiovascular physical exams were significant for an holosystolic murmur in the aortic area. Echocardiographic finding revealed an unicuspidal aortic valve with severe stenosis associated with ascending aorta aneurysm and Standford A dissection. An emergent Bentall procedure was performed with clinical improvement.
