Papillary muscle rupture is a rare mechanical complication of acute myocardial infarction but with a high risk of mortality. It occurs more frequently in inferior infarctions and is not directly related to the size of the infarction.
We present the case of a 79-year-old woman admitted for acute myocardial infarction with ST segment elevation in the inferior wall complicated by posterior papillary muscle rupture and causing severe acute mitral valve insufficiency, managed surgically. This case shows the correlation between clinical manifestations, electrocardiogram, cardiac catheterization, transthoracic echocardiogram, transesophageal echocardiogram and pathological evaluation.
Key words: myocardial infarction, papillary muscle, mitral valve insufficiency, echocardiography, cardiac catheterization.
